Abstract: It is a central aim of health policy initiatives to reduce the income related inequality in health, which seems to be relatively high in Denmark as compared to other European countries. For practical matters income related health inequality is frequently measured on an ordinal scale in the form of self-assessed health (SAH) with five response categories ranging from »very good« to »very bad«. For many purposes it is necessary to transform this measure to a cardinal measure, which may be done by applying scores calculated in external studies. The choice of instrument may potentially influence the results obtained. The present study investigates the sensitivity of measured income related health inequality to the choice of instrument, as external 15D and HUI3 scoring instruments are applied to Danish data. Using interval regression, concentration indices measuring income related health inequality are obtained, and these measures are further decomposed into contributions from socioeconomic and demographic individual characteristics. These contributions are further disentangled into regressor and elasticity contributions. It is concluded that while the basic regression results do not vary across the 15D and HUI instruments, the ability to explain income related health inequality as well as the distinction between regressor and elasticity effects on inequality vary across instruments.
